This framework was developed to provide an appropriate set of indicators to monitor progress towards meeting the three objectives of the National Strategic Framework for Chronic Conditions in a consistent and comparable manner. A framework of standardised indicators provides the signposts that are needed by partners to measure key aspects of chronic condition prevention and control. Such frameworks are recognised as important elements of health-care improvement.
